PROCEDURE FOR ENROLLMENT

First and most important step: Meet and Greet! An appointment will be scheduled for us to meet both parent and child.

Completed Enrollment Package includes:

1. A completed enrollment application form

2. Signed Hand book (last page)

3. Florida Certificate of Immunization (Form 680)

4. Current Physical Examination (DH Form 3040)

5. Copy of birth certificate
6. Copy of Parent ID on file
7. Signed Flu brochure

8. MSD provided ‘Know your Child care facility” brochure

9. Discipline Policy signed

10. Deposit as required

11. All paperwork MUST be turned in before attendance
